Lilly to present ACHIEVE-4, its largest and longest Foundayo study in type 2 diabetes, at EASD 2026

Lilly will present ACHIEVE-4 at the EASD meeting on 1 October 2026: the largest and longest trial of its daily pill Foundayo (orforglipron) in type 2 diabetes. In people with diabetes plus obesity or overweight at raised heart risk, the pill was non-inferior to insulin glargine. Heart events trended lower, but the confidence intervals are wide.

• MACE-4 (heart death, heart attack, stroke, or hospitalisation for unstable angina): 16% lower risk, hazard ratio 0.84 (95% CI 0.59–1.20).
• MACE-3 (without angina): 23% lower risk, hazard ratio 0.77 (95% CI 0.52–1.13).
• Both intervals cross 1.0, so the trial does not prove a heart benefit; it shows no sign of harm against a standard injectable.
• Foundayo is a small-molecule GLP-1 pill taken without food or water restrictions.
