Current obstacles

Biomarker data heterogeneityDifferent medical systems measure different markers using different methods. Reconciling technical and definitional mismatches is a major challenge.
Proving causation vs. correlationBiomarkers correlate with aging, but changing them may not extend life. Proving 20-year outcomes from one-year trials requires statistical extrapolation.

Physics limits

Age is heterogeneousAging involves hundreds of processes across different organs. No single score can capture all; the IC score is necessarily a simplification.
Prediction horizon is longForecasting health 20 years ahead requires assuming the future resembles the past. Therapies, lifestyles and environments all change, making long-term predictions inherently uncertain.

Combining diverse datasets

Stanford integrates health records and biomarker data from multiple institutions. Measurement differences are reconciled into one unified database for analysis.

Composite score

One number for healthspan

Predictive biomarkers are combined into one Intrinsic Capacity score ranging from worst to best health via statistical models.

Intervention trial

Testing whether the score is actionable

Participants follow lifestyle changes while health markers are tracked with home devices. Does improving the score correlate with real health gains?
