Current obstacles

Chronic rejectionNine months of function is encouraging but falls short of human kidney lifespans (15–20 years with immunosuppression). Longer-term trials are essential.
Patient age and comorbiditiesMost trial candidates are age 50+, dialysis-dependent, and on kidney transplant waiting lists, with limited time to show durability.
Immunosuppression toleranceLong-term immunosuppression carries risks of infection and malignancy. The right balance of suppression level and monitoring is not yet defined.

Physics limits

Donor genetic variabilityPigs, like humans, carry genetic variation. Identical gene edits cannot guarantee identical immune outcomes across all pig lines or all human recipients.
Surgical healing timelinesAny transplant must heal surgically and immunologically. Xenografts may heal slower or with more inflammation than allografts due to residual incompatibilities.

Seven human immune genes suppress complement and regulate inflammation to prevent rejection.

Inactivate endogenous retroviruses

PERV sequences are inactivated to eliminate zoonotic infection risk from porcine retroviruses.
