H3-22S fails; payload adapter defects prevent orbit insertion

H3-22S carrying Michibiki-5 QZSS failed Dec 22 when manufacturing defects in payload adapter damaged mounting structure after fairing separation. Second-stage engine shut down prematurely, depleting fuel and preventing orbit insertion. JAXA investigated manufacturing process.
