Physics limits
- PAI-1 also stops bleedingPAI-1 keeps clots from dissolving too early. People with no working PAI-1 have a bleeding disorder, so a drug can only lower it partly, not remove it.
- The genetic clue comes from one small groupThe lifespan link rests on 177 people from one Amish community. A drug taken in adulthood may not match a lifelong genetic change.
- Old scar tissue is slow to reverseScarring diseases build stiff collagen over years. Blocking a driver can slow new scarring, but existing scar tissue clears slowly, if at all.
How it works

PAI-1 rises with age
PAI-1 stops the body's clot-dissolving enzymes. Levels climb with age, obesity and scarring diseases, and worn-out cells release lots of it.
A natural experiment
Amish carriers of one broken PAI-1 gene have about half the normal level and lived about 10 years longer in a 2017 study.
A daily pill to lower it
MDI-2517, a small molecule, blocks PAI-1 to mimic that half-loss; Phase 1 found it safe with once-daily dosing.
