Pittsburgh biotech that grows new organ tissue inside a patient's own lymph nodes. Its lead therapy injects donor liver cells into lymph nodes to grow a second, small liver in people with end-stage liver disease.

## In brief

- Uses lymph nodes as living bioreactors: donor cells injected there grow into working ectopic liver, without major surgery.
- Phase 2a in end-stage liver disease at Houston Methodist; first cohort cleared for dose escalation in March 2025.
- Same idea in preclinical work for thymus (immune aging), pancreatic islets and kidney.

## Programs

- [LYG-LIV-001 ectopic liver](https://technooptimist.io/longevity/lyg-liv-001.md) (Longevity): Pilot; next: Cohort 2: 150M cells across 3 lymph nodes. Phase 2a dose escalation: cohort 1 (4 patients) cleared; higher doses across more lymph nodes next.

## Technical approach

- **Lymph node as bioreactor: Grow tissue where it has blood and room**: Lymph nodes are well supplied with blood and can expand; liver cells placed there multiply and form functioning liver tissue away from the scarred native organ.
- **Donor organs stretched: One liver for dozens of patients**: Cells come from donated livers that missed a transplant match; one liver holds hundreds of billions of hepatocytes, enough for 50+ patients (company estimate).
- **Outpatient delivery: A 10-minute endoscopic injection**: Cells are injected through an endoscope with ultrasound guidance into nodes near the gut, under light sedation, instead of a transplant operation.

## Open problems

- **How much liver is enough** (active research): The company targets a 10–20% boost in liver function; whether ectopic tissue in humans grows that much is what the dose-escalation trial must show.
- **Immune rejection** (active research): Donor cells can be rejected; with iTolerance the company is testing a local tolerance gel to avoid lifelong immune-suppressing drugs.
- **Kidney plumbing** (open): Ectopic kidneys can form, but draining their urine out of a lymph node is unsolved, keeping that program early.
